Registering for the symposium will automatically grant you membership to the SMS. Please note that we will keep the information that you supplied in the above form for as long as you are a member. We will not hold any card information, and the fee will be a one-time fee that can be renewed if you choose to after the time frame has elapsed. 

Deadline for conference registration: Friday 15th November, 5pm
